For actual hand-to-hand or melee weapon fighting? Gods so many contenders I'm not sure I'd be able to nail down one. Probably something Jet Li or Hong Kong cinema in general though.
Gun fights? The Hospital/Elevator scene in Hard Boiled just remains king. All in one take. Blows my mind.
Honorable mention for gun fights is the cantina scene in Desperado.

The Raid movies. Action/fighting galore.
When the brothers take on the one guy is pretty epic from part one.

Also the hotel scene from the under rated HAYWIRE
Gina is awesome in this movie.
