Mortal Kombat Annihilation
I will also point out that King of Fighters is a shit-tier adaptation, even worse than Mario Brothers because it's so damn boring and mind numbingly stupid. I'd rather be waterboarded with piss than ever watch it again. However, I do not love King of Fighters as a franchise so MK:A is my official answer.

Worst adaptation of a book I loved: World War Z. Movie would have been fine on its own had it not had that title or connection to the book. The movie has nothing in common with the book other than the vaguest of connections and was incredibly frustrating for a fan of the book. Different kind of zombies, different story, different everything that felt like a betrayal and butchering of source material I loved. For that reason it was the worst adaptation of any source material I loved. Period.
A terrible adaptation I loved (because it was so awful that it was funny):
DOA: Dead or Alive. I somehow saw this in a theater during its brief run and it was hilariously bad. The best acting in it was Kevin Nash playing.. an aged pro wrestler. It's hard to track down now, but it lives almost rent free in my brain.
